The top picture is what I received, you should receive just that, but you can always return it within 30 days if it's not what you want.

Q: The "Product Description" says both "Compatible With: Gas or diesel fuel" and "Not for use with gasoline." What gives?

The overall dimensions are 18 ½" L x 14" W x 12" H. There is not a provision for a fuel gauge. This tank has a visual gauge in the fuel cap.
